Transition Discoveries: Community Hub

Connect to a network of like-minded individuals, share information, have meaningful conversation, and spend an hour building relationships that will better connect people!

This conversation will include TD team members: self-advocates, adult allies, professionals, and young people with disabilities.

The primary focus of Transition Discoveries is to empower communities to share leadership and collaboration to plan for life after high school with young people with disabilities, their families, and stakeholders.
